We understand that having someone share your home can be a very personal matter. For this reason, our 24 hour live in care team pre-select care professionals to match our staff sympathetically with your situation and personality, as well as your clinical needs.
We also ensure that all our support workers are fully briefed on your expectations and personal preferences.
Hospital to Home Service
In most cases, end of life support is required for clients who have been discharged by the hospital or care facility as they have made chosen their preferred of care is to remain at home. Tordan Healthcare, on receiving a request to support a client discharge, is able to implement a rapid response care needs and support plan to ensure a smooth transition between discharge from hospital and arrival at home, while providing support for the client and their families.
Our dedicated Case Managers and Care Managers work with CCG’s, hospitals, GP and others healthcare professionals to ensure that care needs assessments, support plan and equipment are in place prior to the client discharge. Our recruitment team work with the client and their families to ensure that care professionals recruited are ‘compatible’ with the client.
We work alongside specialist organisations such as Macmillan Cancer Support, Marie Curie and our care professionals receive specific training around the needs of each individual client to ensure the best possible end of life and palliative care.
Tordan Healthcare also provides palliative support to clients who are not receiving curative treatment but have not yet reached ‘end of life’ stage.
