The Company

HFH Healthcare is a complex healthcare service providing bespoke packages of care to highly dependent adults & children living at home with complex conditions requiring specialist care. Our aim at HFH Healthcare is to provide our clients with care teams of the highest calibre.

The Role

We are looking to recruit a dedicated Care Coordinator to work closely within the Care team to ensure client care packages are appropriately staffed and managed. Our in-house clinical team of nurses provide the specific training; however, you will need to ensure the care team are appropriately allocated. You will be managing a case load, being the first point of contact by telephone for the clients and care teams and therefore professionalism is key. This is a complex role in which you need solid client management experience to resolve daily challenges.

Ideal Candidate

To be successful you will need to be solution focused individual, with the ability to work effectively within a fast pace environment as part of a team as well as an individual. Working under time pressure. You will be responsible for dealing with any changes to the rota ensuring sufficient cover for your client is provided at all times.

- Healthcare coordinator experience

- Ability to manage a team of healthcare workers

- Experience of managing staff rotas

- Experience of working with a database/CRM system

- Able to work under pressure and work to tight deadlines

- Able to multi-task and be organised

- Resilient and good interpersonal skills

- Healthcare background desirable
