Support can include:

All support assistants have 24 hours access to brain injury library provided by Home Care Preferred as well as receiving continued support through attendance to Acquired Brain injury, refresher training, seminars and conferences.
All support assistants receive regular review visits from the Brain Injury management team at the beginning of a new package of care as well as regular supervisions and appraisals. Our experience has proved that good communication and support is key to ensure positive outcomes.
Our service is flexible, personalised and we always remember that each client has individual needs. A dedicated team leader is available to them as well as a 24 hour emergency on-call service. Clients have regular reviews, at least every 3 months which ensure there is true satisfaction with our service. It is imperative we are meeting the aspirations of our clients.
The Home Care Preferred Brain Injury Service will work with local authorities, NHS, case managers and insurance companies, as well as other health professionals such as: Speech and Language Therapist, Social Worker, Nurses, Physiotherapists, Occupational Therapists, Clinical Psychologist creating personalised packages to meet the unique needs of the individuals we support.
Meeting Your Needs
Our support assistants are trained to deliver the support that is needed. Our brain injury support service can help people with daily tasks, like getting out of bed and dressed, bathing, and of course household tasks, like cleaning and preparing meals. Our specialist brain injury support assistants are there to help people get around their homes often because reduced mobility is an issue.
At Home Care Preferred we design an individualised person centred care and assessment plan, utilising BINI tool, developed at request of department of health [1] to identify personalised need of people who have a suspected or diagnosed acquired brain injury.
Our support assistants are trained to deliver the support that is needed Such as:
Managing and maintaining nutrition,
Being appropriately clothed
Managing continence
Developing and maintaining family or other personal relationships
Maintaining personal hygiene
Being able to make use of the home safely
Making use of necessary facilities or services in the local community
Companionship and socialising
Maintaining a habitable home environment
Assisting and engaging in work, training, education or volunteering
Assisting with Moving and handling when mobility is an issue
Support from a handyman, driver or gardener
Outings, days out and holidays
Vocational skills development and participation in formal educational programmes
Assistance with caring for PEG feeds, Catheters, Medical gasses, Stoma and Catheter, care, Oxygen therapy, Nebulisers, tracheostomy and Non-Invasive positive pressure ventilation (NIPPY)

Home Care Preferred is able work with residential or nursing homes or those who are in hospital. This support can be based at the care home or hospital or in the form of supporting the client to access to the community.
Working with Case Managers
Home Care Preferred offers Case Managers a dedicated contact person to keep you up to date with your clients’ progress, you can rest assured that you always have the information you want.
We tailor our recruitment process to the individual client’s needs, ensuring the best matched relationship and their support worker.
We review our support plans and have regular consultations with an independent clinical Neuropsychologist who has extensive expertise in supporting people with life after brain injury.
Home Care Preferred have the facility to bring together a full team of health care professionals and this can include: Physiotherapists; Speech and Language Therapists; Occupational Therapists. Our Brain Injury Team Leaders and Support Assistants will assist Case Managers to maximise their clients’ rehabilitation.
We take seriously and understand the legal documentation requirements and the necessity to ensure goals, challenges and risks within a rehabilitation programme are being met
Home support is also widely available across London to support people living in their own homes. We can set up home support virtually anywhere in response to need. Each person will be supported to develop a person-centred support plan which embraces their lifestyle choices whilst ensuring their individual support needs are met.

Home Care Preferred Neurological Support Service offers the highest quality personal care and support to enable people to remain in their own home with dignity, independence and control over their lives.
Our person-centred care package focuses on client’s background, preferred lifestyle and their aspirations. Support plans are regularly reviewed to ensure each client’s changing needs are met.
Our support assistants are trained in the client’s individual condition and have the knowledge to provide the care and assistance to ensure appropriate care is implemented. As always, our clients are encouraged to remain as independent as their condition allows.
We often work closely with a wider team of health care professionals and our team leaders will liaise with those involved to ensure the best outcomes for clients.
