ABOUT THE LONDON CLINIC

Established in 1932 and situated in the heart of London’s medical community, The London Clinic is one of the UK’s largest private hospitals, dedicated to providing the best, personalised healthcare with a breadth of surgical and medical expertise.

- Our main hospital building spans Harley Street and Devonshire Place, looking out onto Marylebone Road. - The Duchess of Devonshire Wing, which was completed and opened in 2010 is located at 22 Devonshire Place. The facility, which is built on eight floors, includes state-of-the-art radiotherapy, chemotherapy suites and 47 dedicated patient bedrooms. The Duchess of Devonshire Wing is also home to the café, on the ground floor, which is open to patients and visitors.

- The London Clinic Eye Centre at 119 Harley Street is a dedicated building which provides consulting, tests and treatments for a range of eye conditions.

- Consulting rooms and diagnostic services at 5 Devonshire Place are provided from this modern, 6 storey building.  Services include blood tests, health assessments and diagnostic imaging.

- Consulting room suites are also located at 145 Harley Street, 120 Harley Street and 116 Harley Street.

- Our administration building is located at 1 Park Square West.

The London Clinic is a charity which is governed by the Chairman and Board of Trustees. They, in turn, delegate the day to day management of the Clinic to the Executive Board of Directors.

The London Clinic provides a number of procedures to assess the health of your heart.  Our diagnostic services facility at 5 Devonshire Place offers a range of heart investigations, helping your cardiologist to develop a treatment programme tailored to your needs.

Electrocardiogram (ECG) and ECG monitoring, including ambulatory 24-hour or 7-day continuous ECG monitoring and event monitoring

SPECIALIST HEART SERVICES
 

CARDIAC IMAGING AT THE LONDON CLINIC
The London Clinic is one of the few private hospitals in central London to offer a comprehensive, multi-modality cardiac imaging service. Our purpose-built stress echocardiography laboratory permits a daily, consultant-led service.

As opposed to invasive techniques, which require catheters to be inserted into the heart, non-invasive tests are safer, easier to perform, and can be used to detect various heart conditions.

These range from reduced blood flow that supplies the heart muscle (known as coronary artery disease) to abnormalities that weaken the capacity of the heart to pump blood.

 

THE CARDIO-ONCOLOGY SERVICE AT THE LONDON CLINIC
WHAT IS CARDIO-ONCOLOGY?
Cardio-Oncology is the management of cardiovascular complications for cancer patients. Cancer treatment (both chemotherapy and radiotherapy) can potentially cause a variety of heart problems.

CARDIAC COMPLICATIONS OF CANCER THERAPY
Common chemotherapies (e.g. Herceptin, anthracyclines/doxorubicin/daunorubicin, R-CHOP, R-ESHAP, Velcade, Avastin, ABVD, FOLFOX, FOLFORI) can damage the heart muscle, coronary arteries and cause irregular heartbeat.

Potential cardiac problems of therapies include heart failure, myocarditis (heart muscle inflammation), arrhythmias (e.g. AF or atrial fibrillation) coronary spasm and heart attacks. 

Radiotherapy can also lead to cardiac complications in some patients, with the most common early problem being an inflammation of the heart muscle and lining (myopericarditis).

Both chemotherapy and radiotherapy can cause cardiac complications months to many years down the line. These include the development of heart failure, heart valve disease and early coronary artery disease (increasing the risk of heart attacks at a younger age).

 

ROLE OF THE CARDIO-ONCOLOGY SERVICE
The Cardio-Oncology service at The London Clinic plays a key role in preventing the development of complications and manging them if they occur. 

Dr Arjun Ghosh is the first Consultant Cardiologist in the UK to be appointed with the specific remit of managing cancer patients with heart problems. He has now brought this expertise to the London Clinic to enable patients to receive world-class Cardio-Oncology care.

PRE-TREATMENT CARDIAC RISK ASSESSMENT
The Cardio-Oncology service can assess the cardiac risk of a patient before the initiation of cancer treatment. The assessment includes a clinical examination followed by specialised cardiac tests e.g. echocardiography (ultrasound scanning of the heart looking at the heart muscle and valves).

Home 24 hour monitoring of the blood pressure and heart rate may be required to further quality cardiac risk. Cardiovascular risk profile blood tests (blood sugar, cholesterol, kidney and liver function) complete the comprehensive assessment.

 

MONITORING DURING CARDIAC TREATMENT
Cancer patients receiving potentially cardiotoxic therapy need to be monitored during the course of their treatment. Monitoring will detect cardiac abnormalities at an early stage to allow the prompt use of cardiac therapy. This may take the form of periodic echocardiography and blood tests.

POST-TREATMENT CARDIAC LATE-EFFECTS
Patients in the Late Effects setting are followed up to detect cardiac complications months to years after the completion of cancer treatment. These reviews include a clinical examination and history, undertaking appropriate ancillary investigations (e.g. ECG and echocardiography) as required.

 RESEARCH IN CARDIO-ONCOLOGY
Dr Ghosh is heavily involved in Cardio-Oncology education and research, having published a number of papers in the field and is regularly invited to speak, nationally and internationally on the topic.

INTERNATIONAL CLINICAL TRIAL FOR BRAIN CANCER
Intra-operative radiotherapy (IORT) is a pioneering radiotherapy technique for patients with GBM (Glioblastoma Multiforme), a brain cancer with a high mortality rate.

The London Clinic is the first and currently only site in the UK to offer IORT for GBM, as part of the INTRAGO II phase 3 clinical trial, and one of 12 international hospitals taking part.

The INTRAGO II trial aims to bridge the inevitable gap between surgery and post-operative radiotherapy, and investigate the benefit of the treatment for glioblastoma, to help patients now and in the future.

NEURO INTRA-OPERATIVE RADIOTHERAPY (IORT)
IORT is delivered with the Carl Zeiss Intrabeam® system - a miniaturised X-ray device which is located within our on-site theatres. During surgery and after the tumour is removed, a high dose of radiotherapy is administered directly to the tumour cavity, aimed at clearing any microscopic disease which may have been left behind. Currently, IORT is only offered as part of a clinical, randomised phase 3 trial.

IORT removes the unnecessary delay in administering radiotherapy treatment to the patient. Radiotherapy is applied following surgery with patient under anaesthetic prior to closure. This technique is safe and allows local dose intensification.

 

WHAT IS GLIOBLASTOMA MULTIFORME (GBM)?
GBM is the most common malignant brain cancer, affecting roughly 2,000 adults every year in the UK, with figures predicted to rise over the years to come.

GBM is primarily treated with a combination of surgery, chemotherapy and external beam radiotherapy. Despite this, due to the high growth rate of cells there is a high chance the tumour can reoccur.

IORT shows great promise in the treatment of glioblastoma. At The London Clinic, a boost dose of up to 30Gy may be delivered, in addition to standard post operative radiotherapy, to better control the cancer cells left behind in the tumour bed, before any cells start to regrow.

 

INTRAGO II, PHASE 3 CLINICAL TRIAL
INTRAGO II is an international, multi-centre phase 3, randomised trial. Patients are randomly allocated between two different trial groups. In one group, the patient will receive the standard care of treatment including radiotherapy and chemotherapy. In the other, the patient will receive the standard care with an additional boost of intra-operative radiotherapy (IORT) delivered during surgery. All patients are carefully followed-up to monitor their condition.

 

WHO CAN APPLY FOR THE CLINICAL TRIAL?
Patients who have a suspected GBM without any previous radiotherapy or chemotherapy, can be considered for the trial. Your doctor will advise you if you are suitable to take part in the INTRAGO II trial. Candidates must be aged 18 or older and younger than 80, and must not have had previous cranial radiotherapy.

THE IORT SERVICE AT THE LONDON CLINIC
The London Clinic supports a comprehensive neurological service, incorporating high-spec linear accelerators for radiotherapy, stereotactic radiotherapy with CyberKnife™, a state-of-the-art intensive care unit (ICU) and a comprehensive rehabilitation and aftercare programme.

Consultant neurosurgeons, Professor Keyoumars Ashkan, Mr Ranj Bhangoo and Consultant Oncologist, Dr Matt Williams, are leading the clinical trial at The London Clinic, and are asking consultant oncologists and neurologists to refer patients for the clinical trial. Participation in this trial reflects our approach to research and innovation, and will provide patients a new treatment option for GBM.

 

INTRA-OPERATIVE RADIOTHERAPY (IORT) FOR BREAST CANCER

INTRA-OPERATIVE RADIOTHERAPY FOR BREAST CANCER – TARGIT IORT
Intra-operative radiotherapy (IORT) is an innovative technique in which low-energy X-ray radiation is used to treat suitable patients affected by breast cancer. At the London Clinic, the Carl Zeiss Intrabeam® system is utilised at the time of surgery to precisely deliver the prescribed radiation dose to the site of the cancer. In most cases, this single one-off treatment replaces the traditional external beam radiotherapy technique, normally requiring 3-5 weeks of therapy over 15-25 separate visits to the hospital.

The Intrabeam® system has been subject to extensive research in order to compare IORT in relation to conventional whole-breast external beam radiotherapy. Between 2000-2012, the TARGIT-A randomised trial recruited over 3000 patients across Europe. Results published indicate that when IORT is given at the time of the surgery the success rate is similar to conventional treatment and notably IORT patients exhibited fewer associated side-effects.

The London Clinic participates in the Targit-R registry which helps ensure that patients are followed-up and safely monitored throughout their treatment and recovery.

 

THE IORT TEAM AT THE LONDON CLINIC
The London Clinic works with leading specialist surgeons and oncologists, fully trained in up-to-date IORT treatment methods. The London Clinic IORT radiotherapy team consists of physicists and radiographers who provide expertise in the scientific and technical delivery of low energy X-rays.

The team supported delivery of the first privately treated breast IORT patient in the UK, were the first site to utilise specialist “flat applicators” for dose delivery in colorectal applications and supported the UK implementation of the INTRAGO II trial for glioblastoma.

The team have also contributed to research and international conferences in this specialist area.

Colorectal cancer is primarily treated with surgery but in some cases, in order to help reduce the chance of recurrence, intra-operative radiotherapy can be given during the operation. The London Clinic works with leading surgeons and oncologists in this field to provide this treatment and were the first in the UK to use the Zeiss Intrabeam system with specifically designed “flat applicators” for colorectal cancer treatments.
